	Online Version For Rating Multiple Providers. An on-line version of the questionnaire that enables you to rate several providers at once, with drop-down lists for answering questions, can be found by clicking here. Please note that it is possible to save the questionnaire on-line at any time by scrolling down and clicking on the Save button at the foot of each of the three pages of the questionnaire, and we strongly advise you to do this frequently in order to avoid losing your work. However, in order to do so you must give your e-mail address at the top of the first page so that we can send you a web address and password to access your work again. It is not necessary to complete “required” questions to use the Save option, but the Save option will not work unless we are given an e-mail address.

Please note this Excel version is NOT an online version, so please be sure to download and save the questionnaire on your PC or laptop before starting to complete it to ensure you do not lose your work. This will also enable you to e-mail it as an attachment to colleagues to complete or review, and e-mail it back to us at surveys@globalcustodian.com when you have completed it.
	PDF For Rating Multiple Providers By Hand. A PDF version that enables you to rate several providers at once, and which can be completed by hand, can be downloaded and printed by clicking here. Please fax the completed questionnaires to us at any of the following numbers: UK 44 (0)20 7900 3093, or UK 44 (0)20 7100 8115, or toll free in the US 866-332-6781. Please note that because faxed versions have to be re-keyed, and often need repair if writing is illegible or “required” questions are left blank, there is a strong possibility that we will not be able to inform your provider that you have submitted the questionnaire as promptly as we can with other versions. It is also possible that we may need to contact you to confirm details or complete “required” questions.
